Welcome to my Nurse page where you will find some information to assist with any medical concerns that may come up during the school year. Please feel free to browse the links to medical documents.
Guidelines for New Registrants
● If you are registering your child for the first time in our school, please be sure to bring in an updated vaccination record--this is Mandatory for registration!
● Please view the link for "New Registration Packet": parents must complete the health assessment document (2 pages)
● New students require a physical exam from within the year as well as a dental exam (see links for physical and dental) these are in the new registration packet as well.
● If your child is coming from out of the U.S., he or she may need a TB (tuberculosis) skin test. Please view the link "Tuberculosis Testing Information" for a list of countries that DO NOT require testing, to note if this pertains to you.
● Any other medical documents that you feel would be beneficial to have at school are always encouraged to help better care for children during school hours
○ *Please note: if medication is required in school, documents must be completed by healthcare provider and parent before medication is brought to school. Additional medication packets are located under Resources' and for your convenience.*
I am happy and proud to call myself the Washington School Nurse. I have been working here since 2021 and worked as an RN for 18 years. It has been quite a wonderful experience thus far! It is my pleasure to care for your children on a daily basis.
